This pharmacological intervention is a combination therapy used as an adjunct to extracorporeal shock wave lithotripsy (ESWL) for the treatment of kidney stones, specifically lower pole caliceal stones. The regimen consists of the intravenous administration of 0.5 liters of 0.9% sodium chloride (normal saline) and 20 mg of furosemide (also known as frusemide), a loop diuretic. Developed and studied by investigators at Monash University, this protocol is designed to induce forced diuresis. By increasing urinary flow and volume, the therapy aims to facilitate the mechanical flushing and gravitational clearance of stone fragments (gravel) from the renal calyces following shock wave fragmentation, particularly when combined with physical maneuvers such as inversion therapy.
