This therapeutic intervention consists of cold (4°C) crystalloid infusions, specifically 0.9% sodium chloride or Ringer's solution, used for the rapid induction of therapeutic hypothermia in stroke patients. Developed and evaluated by University Hospital Heidelberg in the iCOOL 2 randomized phase 2 trial, the protocol involves the intravenous administration of 2 liters of the chilled solution over a 30-minute period. The primary goal is to lower body and brain temperature quickly to reduce metabolic demand, mitigate secondary brain injury, and decrease brain swelling associated with acute ischemic or hemorrhagic stroke. Unlike traditional pharmacological agents, this intervention works through physical cooling rather than binding to specific biological receptors.
