Sodium nitrite + N-acetylcysteine is an investigational combination therapy being developed by the University of Alabama at Birmingham for the treatment of hypertension and cardiovascular diseases. When administered sublingually, the two components react in the sublingual space to generate S-nitroso-N-acetylcysteine (SNOAC), a low-molecular-weight S-nitrosothiol that acts as a nitric oxide (NO) donor. SNOAC rapidly diffuses into the systemic circulation, increasing plasma S-nitrosothiol levels and promoting vasodilation, which reduces systolic and diastolic blood pressure. This approach is intended to provide the benefits of nitric oxide therapy while avoiding the drug tolerance typically associated with organic nitrates like nitroglycerin.
