Sodium polystyrene sulfonate + calcium polystyrene sulfonate is a combination of two cation-exchange resins, each used as a potassium-binding agent for the treatment of hyperkalemia (high blood potassium), typically in patients with chronic kidney disease. Sodium polystyrene sulfonate acts by exchanging sodium for potassium ions in the colon, leading to intestinal elimination of potassium, though it may also bind calcium and magnesium. Calcium polystyrene sulfonate works by exchanging calcium for potassium and is also effective in lowering serum potassium. Both have a delayed onset of action and are primarily used for longer-term potassium management rather than emergency hyperkalemia. The main adverse effects are gastrointestinal, mainly constipation, although rare but significant complications such as intestinal necrosis have been reported, especially with sodium polystyrene sulfonate. This combination is theoretically possible as both are used for the same indication, though marketed fixed-dose combination products are uncommon; the combination may be employed in practice to individualize therapy and mitigate sodium load or calcium changes in specific patients[1][2][4][6][9].
