Overview

This is a four-drug investigational antiviral combination consisting of sofosbuvir, daclatasvir, CDI-31244 (also known as CC-31244), and asunaprevir. All components are direct-acting antivirals targeting hepatitis C virus (HCV) replication through distinct mechanisms: - **Sofosbuvir** is a nucleotide analog inhibitor of the HCV NS5B RNA-dependent RNA polymerase. - **Daclatasvir** is an inhibitor of the HCV NS5A protein, essential for viral replication and assembly. - **CDI-31244 (CC-31244)** is an investigational non-nucleoside inhibitor (NNI) that targets the HCV NS5B polymerase at a different site than sofosbuvir, designed to have broad-spectrum activity against all major HCV genotypes and a high barrier to resistance[7]. - **Asunaprevir** is an inhibitor of the HCV NS3/4A protease. The rationale behind combining these agents is to achieve potent suppression of viral replication by simultaneously inhibiting multiple steps in the viral life cycle. This approach aims to increase cure rates, reduce treatment duration, and minimize resistance development. While combinations such as sofosbuvir/daclatasvir are approved and widely used for chronic hepatitis C infection[1][2][4], adding experimental agents like CDI-31244 represents ongoing research into ultrashort or more effective regimens[7].
