Overview

This is a combination antiviral regimen consisting of three agents—sofosbuvir, ribavirin, and peginterferon alfa-2—used primarily for the treatment of chronic hepatitis C virus (HCV) infection. Sofosbuvir is a nucleotide analog inhibitor that targets the HCV NS5B RNA-dependent RNA polymerase, acting as a chain terminator to prevent viral replication. Ribavirin is a nucleoside analog with multiple proposed mechanisms including inhibition of inosine monophosphate dehydrogenase (IMPDH), induction of lethal mutagenesis in viral genomes, and immunomodulatory effects favoring Th1 responses. Peginterferon alfa-2 (either 2a or 2b) is a long-acting form of interferon alpha that induces expression of interferon-stimulated genes with broad antiviral activity by enhancing host immune response against HCV. This triple therapy was historically used for genotypes 1 and 4 HCV infections in both treatment-naive and experienced patients[4][5][7]. The regimen has largely been replaced by newer direct-acting antivirals but remains an option in certain clinical scenarios or resource-limited settings[4][5].
