Drug intelligence / Profile preview
Sofosbuvir/velpatasvir (brand name Epclusa) is a fixed-dose combination small molecule antiviral drug developed by Gilead Sciences for the treatment of chronic hepatitis C virus (HCV) infection. It combines two potent direct-acting antivirals: sofosbuvir, a nucleotide analog inhibitor of the NS5B RNA-dependent RNA polymerase, and velpatasvir, a pan-genotypic inhibitor of the NS5A protein. This combination is the first all-oral, single-tablet regimen approved for the treatment of adults and pediatric patients (3 years and older) with HCV genotypes 1 through 6. It is indicated for use in patients without cirrhosis or with compensated cirrhosis, and in combination with ribavirin for those with decompensated cirrhosis. Additionally, it is being investigated in Phase 4 clinical trials for use as post-exposure prophylaxis (PEP) in healthcare workers following high-risk exposures such as needlestick injuries.
