This is a non-standard, theoretical combination of two fixed-dose antiviral hepatitis C therapies—Epclusa (sofosbuvir/velpatasvir) and Zepatier (elbasvir/grazoprevir). Each product is itself a combination of direct-acting antivirals targeting different proteins essential for hepatitis C virus (HCV) replication. Sofosbuvir is an NS5B RNA-dependent RNA polymerase inhibitor, velpatasvir and elbasvir are NS5A inhibitors, and grazoprevir is an NS3/4A protease inhibitor. These agents act synergistically to inhibit HCV replication at multiple stages of the viral life cycle. Epclusa covers all six major HCV genotypes, while Zepatier targets genotypes 1 and 4[2][3][5]. There are no clinical guidelines or approvals for using both combinations together.
