Overview
Sofosbuvir + velpatasvir + ribavirin + peginterferon is an investigational salvage combination regimen being evaluated for the re-treatment of chronic hepatitis C virus (HCV) infection, particularly in patients who have failed or relapsed after prior direct-acting antiviral (DAA) therapy. This quad-therapy approach combines three small molecule antivirals with an immune-modulating biologic. Sofosbuvir acts as a nucleotide analogue inhibitor of the HCV NS5B RNA-dependent RNA polymerase, while velpatasvir targets the NS5A replication complex protein. Ribavirin is a broad-spectrum antiviral that interferes with viral RNA synthesis and induces viral mutagenesis. Peginterferon (pegylated interferon) provides an immune-modulating effect by stimulating innate antiviral pathways. This regimen is primarily studied in resource-limited settings as an empiric re-treatment strategy when newer second-line DAA combinations, such as those containing voxilaprevir, are unavailable.
