Sorafenib + interferon alfa-2a is a combination therapy consisting of sorafenib, a small molecule multikinase inhibitor, and interferon alfa-2a, a recombinant cytokine. Sorafenib acts by inhibiting several kinases involved in tumor cell proliferation and angiogenesis, including RAF kinases, vascular endothelial growth factor receptors (VEGFR), and platelet-derived growth factor receptor (PDGFR). Interferon alfa-2a induces antitumor and immunomodulatory effects by promoting the transition of tumor-associated macrophages (TAMs) from an immunosuppressive M2 phenotype to a pro-immunogenic M1 phenotype, and by increasing intratumoral CD8+ T-cell infiltration. This combination exhibits synergistic antitumor activity, particularly studied in advanced hepatocellular carcinoma (HCC), renal cell carcinoma (RCC), and melanoma, with demonstrated improvement in tumor control rates and survival in clinical trials. Sorafenib is primarily developed by Bayer and Onyx Pharmaceuticals, while interferon alfa-2a is developed by Roche. The combination is administered orally (sorafenib) and typically subcutaneously (interferon alfa-2a).
