This combination regimen consists of the oral multikinase inhibitor sorafenib and hepatic arterial infusion chemotherapy (HAIC) using oxaliplatin and 5-fluorouracil (5-FU). It was evaluated in a Phase 2 clinical trial led by Peking University for the treatment of hepatocellular carcinoma (HCC) with major portal vein tumor thrombosis (PVTT). Sorafenib acts by inhibiting tumor angiogenesis and proliferation through the blockade of VEGFR, PDGFR, and the RAF/MEK/ERK pathway. Oxaliplatin is a platinum-based agent that causes DNA cross-linking, while 5-fluorouracil is an antimetabolite that inhibits thymidylate synthase, interfering with DNA synthesis. The HAIC delivery method allows for high local concentrations of chemotherapy within the liver while reducing systemic side effects.
