Overview

This entry refers to the standard-of-care rhythm control arm in the Flec-CAD Phase 4 clinical trial sponsored by Universitaire Ziekenhuizen KU Leuven. In this trial, patients with atrial fibrillation and stable coronary artery disease are randomized to receive either flecainide or a standard rhythm-control drug, which is defined as either sotalol or amiodarone based on physician preference. Sotalol is a non-selective beta-adrenergic receptor blocker that also possesses Class III anti-arrhythmic properties by blocking the rapid component of the delayed rectifier potassium current (IKr). Amiodarone is a broad-spectrum Class III anti-arrhythmic agent that exhibits electrophysiological characteristics of all four Vaughan Williams classes, including sodium channel blockade (Class I), non-competitive anti-sympathetic action (Class II), potassium channel blockade (Class III), and calcium channel blockade (Class IV). Both agents are used to maintain sinus rhythm in patients with symptomatic atrial fibrillation.
