This combination therapy is an experimental regimen being evaluated in the Phase II COLOSOTO (ENGIC 01) clinical trial, sponsored by the Federation Francophone de Cancerologie Digestive (FFCD). It consists of sotorasib (a small molecule KRAS G12C inhibitor), panitumumab (a monoclonal antibody targeting EGFR), and 5-fluorouracil (a pyrimidine analog chemotherapy). The regimen is specifically designed as a first-line treatment for frail or elderly patients (aged 75+ or with a WHO performance status of 2) who have advanced unresectable colorectal adenocarcinoma harboring the KRAS G12C mutation and are ineligible for standard intensive doublet or triplet chemotherapy. By combining targeted inhibition of the KRAS G12C mutation and the EGFR pathway with low-intensity chemotherapy, the treatment aims to overcome resistance mechanisms and provide a tolerable yet effective option for this specific patient population.
