Drug intelligence / Profile preview
sotorasib + panitumumab + irinotecan + fluorouracil + leucovorin is an investigational combination regimen consisting of the KRAS G12C inhibitor sotorasib (Lumakras), the anti-EGFR monoclonal antibody panitumumab (Vectibix), and the FOLFIRI chemotherapy regimen (irinotecan, fluorouracil, and leucovorin). This combination is developed for the treatment of patients with KRAS G12C-mutated metastatic colorectal cancer (mCRC). Sotorasib selectively and irreversibly binds to the KRAS G12C mutant protein, locking it in an inactive GDP-bound state to inhibit downstream oncogenic signaling. Panitumumab binds to the epidermal growth factor receptor (EGFR), blocking ligand binding and downstream signaling, which helps overcome EGFR-mediated feedback reactivation of the MAPK pathway often triggered by KRAS G12C inhibition. Irinotecan is a topoisomerase I inhibitor that prevents DNA replication, fluorouracil is a thymidylate synthase inhibitor that disrupts DNA synthesis, and leucovorin acts as a biochemical booster to enhance the efficacy of fluorouracil. Together, this combination provides a synergistic approach by simultaneously targeting the mutated KRAS driver, blocking EGFR-mediated resistance pathways, and delivering cytotoxic chemotherapy.
