Spartalizumab + pemetrexed + cisplatin is a three-drug combination regimen consisting of spartalizumab, a PD-1–blocking monoclonal antibody, pemetrexed (an antifolate antineoplastic agent), and cisplatin (a platinum-based DNA-damaging cytotoxic drug). Spartalizumab enhances antitumor immune responses by inhibiting the interaction between PD-1 and its ligands, restoring T-cell activity. Pemetrexed inhibits multiple folate-dependent enzymes involved in nucleotide biosynthesis, thereby impairing DNA and RNA synthesis in rapidly dividing tumor cells. Cisplatin creates DNA crosslinks and induces apoptosis by interfering with DNA replication and transcription. This combination is under investigation as first-line therapy for advanced non-small cell lung cancer (NSCLC) and other solid tumors, aiming to synergistically promote tumor cell death through immune activation and direct cytotoxicity. In clinical trials, this combination has shown antitumor activity and a manageable safety profile comparable to existing immunotherapy plus chemotherapy regimens[1].
