Overview

A combination of spironolactone, an aldosterone receptor antagonist that acts as a potassium-sparing diuretic, and carvedilol, a beta-blocker that also has alpha-1 adrenergic blocking properties. This combination is commonly used in the treatment of heart failure, where spironolactone prevents sodium reabsorption and potassium excretion in the kidney while carvedilol reduces heart rate, blood pressure, and cardiac workload. Clinical evidence from the COPERNICUS study shows that carvedilol remains efficacious when added to background spironolactone therapy in patients with severe heart failure. However, this combination requires careful monitoring of serum potassium levels, blood pressure, and renal function.
