spironolactone + losartan is a combination therapy being evaluated by the Tehran University of Medical Sciences for the treatment of diabetic nephropathy in patients with type 2 diabetes. Spironolactone is a small molecule mineralocorticoid receptor antagonist (MRA) that blocks the effects of aldosterone, while losartan is a small molecule angiotensin II receptor blocker (ARB) that targets the type-1 angiotensin II (AT1) receptor. The combination aims to provide a more complete blockade of the renin-angiotensin-aldosterone system (RAAS), specifically addressing the phenomenon of 'aldosterone escape' which can occur during monotherapy with ACE inhibitors or ARBs. By reducing aldosterone-mediated damage and angiotensin II-induced vasoconstriction and inflammation, the therapy is intended to decrease urinary albumin excretion and slow the progression of renal impairment.
