SPIOMET is a fixed-dose oral combination therapy consisting of three small molecule drugs—spironolactone (a mineralocorticoid receptor antagonist with anti-androgenic properties), pioglitazone (a thiazolidinedione insulin sensitizer acting as a peroxisome proliferator-activated receptor gamma agonist), and metformin (a biguanide insulin sensitizer). This combination is under clinical investigation for the treatment of polycystic ovary syndrome (PCOS) in adolescent girls and young adult women. The rationale for combining these agents is to target multiple pathophysiological aspects of PCOS, including hyperandrogenism, insulin resistance, and central adiposity. Spironolactone reduces androgen effects; pioglitazone improves insulin sensitivity via PPARγ activation; metformin decreases hepatic glucose production and increases peripheral glucose uptake. Together, they aim to normalize ovulation rates and improve metabolic-endocrine parameters more effectively than monotherapy or oral contraceptives[1][5][7].
