SRP + alendronate

SRP + alendronate refers to the combination therapy of scaling and root planing (SRP), a non-pharmacological dental procedure for treating periodontitis, with adjunctive administration of the drug alendronate. Alendronate is a bisphosphonate that inhibits osteoclast-mediated bone resorption, thereby preventing bone breakdown and increasing bone density. In this combination, alendronate can be administered either locally (directly into periodontal sites) or systemically (oral or intravenous). The addition of alendronate to SRP has been shown in meta-analyses to improve clinical outcomes in periodontitis patients compared to SRP alone—specifically by increasing clinical attachment level (CAL), reducing probing depth (PD), and enhancing bone defect fill. Locally delivered alendronate appears more effective than systemic administration for these endpoints. However, systemic use carries risks such as osteonecrosis of the jaw[1][2][4].
