SRP + aloe vera refers to the combination therapy of scaling and root planing (SRP), a standard non-surgical periodontal treatment, with adjunctive local application of aloe vera gel or hydrogel. This combination is used primarily in the management of chronic periodontitis. Aloe vera contains biologically active substances such as mannose 6-phosphate, carboxypeptidase, glutathione peroxidase, superoxide dismutase, acemannan, aloin, and others that confer anti-inflammatory, antibacterial, antifungal, antioxidant properties and promote wound healing. When used as an adjunct to SRP—typically by subgingival delivery into periodontal pockets—aloe vera has been shown in multiple clinical studies and meta-analyses to significantly improve clinical parameters such as probing depth (PD), clinical attachment level (CAL), plaque index (PI), gingival index (GI), and pocket probing depth (PPD) compared to SRP alone[1][5][8]. The mechanism involves inhibition of matrix metalloproteinases (MMPs) like MMP-2/9 which are implicated in tissue destruction during periodontitis; reduction of inflammation via COX-1/COX-2 inhibition; antimicrobial effects against oral pathogens; promotion of collagen formation; and enhanced wound healing[5]. No significant adverse effects have been reported for this combination therapy.
