St. Thomas' blood cardioplegia is an extracellular, depolarizing cardioplegic solution used to induce and maintain cardiac arrest during cardiac surgery, protecting the myocardium from ischemic injury. Developed at St. Thomas' Hospital in London, the solution typically contains high concentrations of potassium (to induce arrest via depolarization) and magnesium (acting as a calcium channel blocker to prevent calcium overload). The "blood" formulation involves mixing the crystalloid St. Thomas' solution with the patient's blood, usually in a 4:1 or 1:4 ratio, to provide oxygen-carrying capacity and buffering during the cross-clamp period. It is widely used in pediatric cardiac surgery and is currently being compared to del Nido cardioplegia in the DESTINY trial sponsored by the University of Birmingham.
