Overview

Standard ART + Mega ART refers to a combination approach in antiretroviral therapy (ART) for HIV infection. "Standard ART" typically denotes the use of a recommended, evidence-based regimen consisting of at least three antiretroviral drugs from at least two different classes, designed to maximally suppress HIV replication and prevent disease progression[1][3][5]. "Mega ART" is not a standard term in clinical guidelines but is sometimes used informally or in research contexts to describe intensified regimens that include more than the usual number of antiretroviral agents—often four or more drugs, sometimes including additional drug classes or agents reserved for highly treatment-experienced patients with multidrug-resistant HIV. The rationale behind combining Standard and Mega regimens would be to achieve greater viral suppression in cases where resistance or advanced disease necessitates an aggressive approach. Mechanistically, these regimens act by targeting multiple stages of the HIV life cycle using various drug classes such as nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs), non-nucleoside reverse transcriptase inhibitors (NNRTIs), protease inhibitors (PIs), integrase strand transfer inhibitors (INSTIs), entry/fusion inhibitors, and others[8][10]. The specific composition depends on individual patient factors including prior treatment history and resistance patterns.
