This combination therapy consists of a low-dose statin (HMG-CoA reductase inhibitor) paired with Armolipid Plus, a multi-ingredient nutraceutical formulation. Armolipid Plus contains red yeast rice (standardized to monacolin K), policosanol, berberine, folic acid, coenzyme Q10, and astaxanthin. The combination is designed to lower LDL cholesterol levels in patients with high-risk coronary artery disease who are intolerant to high-intensity statin therapy. The mechanism involves the synergistic inhibition of cholesterol synthesis (via monacolin K and statins), inhibition of cholesterol absorption, and upregulation of LDL receptors (via berberine). It was evaluated in the ADHERENCE trial conducted by researchers at Sapienza University of Rome.
