STEMVAC + chemotherapy is an investigational combination therapy being evaluated for the treatment of metastatic triple-negative breast cancer (TNBC). The primary component, STEMVAC, is a multi-antigen DNA plasmid vaccine developed by the University of Washington Cancer Vaccine Institute. It is designed to encode Th1-selective epitopes from five proteins associated with cancer stem cells: endoglin (CD105), Y-box binding protein 1 (YB1), SRY-box transcription factor 2 (SOX2), cadherin 3 (CDH3), and MDM2 proto-oncogene. The vaccine aims to stimulate a Th1-mediated immune response to target and eliminate cancer stem cells, which are often resistant to standard therapies and contribute to tumor recurrence. In clinical trials, STEMVAC is administered intradermally alongside sargramostim (GM-CSF) as an adjuvant and in combination with standard-of-care chemotherapy regimens, which provide direct cytotoxic effects against the bulk tumor mass.
