Overview

A combination therapy comprising **succinylcholine**, **cisatracurium**, **neostigmine**, and **atropine** is used perioperatively or in intensive care settings to achieve rapid-onset muscle relaxation and subsequent reversal, mainly during surgical anesthesia or mechanical ventilation. Succinylcholine is a depolarizing neuromuscular blocker inducing rapid skeletal muscle paralysis, commonly for intubation; cisatracurium is a non-depolarizing neuromuscular blocker used for sustained muscle relaxation. Neostigmine, an acetylcholinesterase inhibitor, reverses non-depolarizing neuromuscular blockade (e.g., from cisatracurium) by increasing acetylcholine at neuromuscular junctions. Atropine, an antimuscarinic agent, is co-administered with neostigmine to counteract its muscarinic (parasympathomimetic) side effects, like bradycardia and excessive secretions. This drug combination, therefore, covers rapid induction, sustained paralysis, safe reversal, and mitigation of cholinergic side effects[1][2][3][4][5].
