PA21 + warfarin refers to the co-administration of sucroferric oxyhydroxide (PA21) and warfarin, primarily investigated in clinical pharmacology studies to assess potential drug-drug interactions. Sucroferric oxyhydroxide is a non-calcium, iron-based phosphate binder used to treat hyperphosphatemia in patients with chronic kidney disease (CKD) on dialysis. Warfarin is a vitamin K antagonist anticoagulant commonly prescribed to CKD patients for the prevention of thromboembolic events. Because iron-based binders can potentially adsorb other medications in the gastrointestinal tract, Phase I studies were conducted to determine if sucroferric oxyhydroxide affects the pharmacokinetics (absorption, distribution, metabolism, and excretion) or pharmacodynamics (International Normalized Ratio, INR) of warfarin. Clinical data generally indicate that sucroferric oxyhydroxide does not have a clinically significant effect on warfarin levels, allowing for their co-administration without specific dose adjustments for the binder's effect.
