Drug intelligence / Profile preview
Xacduro + imipenem is a co-administration regimen combining Xacduro (a co-packaged formulation of the beta-lactam antibiotic sulbactam and the novel broad-spectrum diazabicyclooctane beta-lactamase inhibitor durlobactam) with the carbapenem antibiotic imipenem (typically administered as imipenem-cilastatin). This combination was evaluated in the pivotal Phase 3 ATTACK trial for the treatment of serious infections, including hospital-acquired bacterial pneumonia (HABP) and ventilator-associated bacterial pneumonia (VABP), caused by carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex (ABC). Sulbactam exhibits intrinsic bactericidal activity against Acinetobacter species by binding to penicillin-binding proteins PBP1 and PBP3, while durlobactam protects sulbactam from degradation by Ambler Class A, C, and D beta-lactamases (such as OXA carbapenemases). Imipenem provides additional antibacterial activity and target redundancy by binding to PBP1a, PBP2, and PBP3, and is protected from carbapenemases by durlobactam, resulting in synergistic bactericidal activity against multidrug-resistant pathogens.
