Overview

This drug is a fixed-dose combination of three antimalarial agents—sulfadoxine, pyrimethamine, and piperaquine. Each component acts via a distinct mechanism to inhibit the growth and replication of Plasmodium species, the causative agents of malaria. Sulfadoxine is a sulfonamide that inhibits dihydropteroate synthase, blocking folate synthesis in the parasite. Pyrimethamine inhibits dihydrofolate reductase, further disrupting folate metabolism essential for DNA synthesis. Piperaquine is a bisquinoline compound that interferes with heme detoxification in the parasite’s food vacuole, leading to toxic accumulation and parasite death. The combination has been studied primarily for intermittent preventive treatment (IPT) of malaria in children and pregnant women in endemic regions. It aims to leverage both the long half-life and broad activity spectrum against resistant strains by combining these drugs[2][1]. While not widely licensed as a fixed combination product, it has been evaluated in clinical trials for efficacy and safety compared to other regimens such as sulfadoxine-pyrimethamine plus amodiaquine or dihydroartemisinin-piperaquine[2][3]. The main indication under investigation is prevention of malaria during pregnancy (IPTp) or seasonal malaria chemoprevention (SMC) in children.
