Overview

Sulfamethoxazole + mesalamine is a combination of two pharmaceutical agents, each with distinct mechanisms and indications. Sulfamethoxazole is a bacteriostatic sulfonamide antibiotic that inhibits folic acid synthesis in susceptible bacteria by blocking dihydropteroate synthase, thereby interfering with nucleic acid and protein biosynthesis essential for bacterial growth[2]. It is primarily used to treat various bacterial infections, often in combination with trimethoprim. Mesalamine (also known as mesalazine) is an aminosalicylate anti-inflammatory drug used to treat mild to moderate ulcerative colitis and maintain remission[1][4][8]. Its mechanism involves inhibition of prostaglandin and leukotriene synthesis, reduction of leukocyte migration, and scavenging of free radicals—actions that reduce inflammation mainly at the site of disease in the colon[4]. There are no widely recognized fixed-dose combinations or clinical indications for using these two drugs together; however, pharmacokinetic studies show coadministration does not result in clinically significant interactions[3][5].
