Sulfamethoxazole + trimethoprim + bromhexine is an approved fixed-dose combination drug developed by DESMA HC Group and marketed under the brand name Balsoprim. This therapeutic combines the synergistic antibacterial effects of co-trimoxazole (sulfamethoxazole and trimethoprim) with the mucolytic properties of bromhexine. Sulfamethoxazole, a sulfonamide, inhibits dihydropteroate synthase, while trimethoprim inhibits dihydrofolate reductase; together, they block successive steps in bacterial folate synthesis, providing broad-spectrum activity against various Gram-positive and Gram-negative pathogens. Bromhexine acts as a secretolytic agent, increasing the production of serous mucus and reducing phlegm viscosity to facilitate expectoration. The combination is primarily indicated for respiratory tract infections where congestion is present, such as bronchitis, but is also used for urinary tract infections, otitis media, and certain gastrointestinal infections like shigellosis.
