Overview

Sulopenem is a synthetic thiopenem antibiotic belonging to the carbapenem class, designed to treat infections caused by multi-drug resistant Gram-negative bacteria. The designation 'CP-70,429 + PF-03709270' refers to the development of the drug in two distinct forms: an intravenous formulation (CP-70,429, the sodium salt) and an oral prodrug formulation (PF-03709270, sulopenem etzadroxil). This dual-delivery strategy was intended to support 'step-down' therapy, allowing patients to transition from hospital-based intravenous treatment to oral therapy at home. Sulopenem exerts its antibacterial effect by binding to and inhibiting penicillin-binding proteins (PBPs), which are essential for bacterial cell wall synthesis. Originally developed by Pfizer for indications such as pneumonia and urinary tract infections, the drug was licensed to Iterum Therapeutics in 2015. Iterum has since focused on complicated and uncomplicated urinary tract infections (cUTI/uUTI) and complicated intra-abdominal infections (cIAI), while development for pneumonia was discontinued.
