BI 1815368 is a fixed-dose combination (FDC) therapy under development by Boehringer Ingelheim, comprising survodutide (BI 456906) and metformin. Survodutide is a dual agonist of the glucagon-like peptide-1 receptor (GLP-1R) and the glucagon receptor (GCGR), designed to mimic the natural hormone oxyntomodulin. By targeting both receptors, it aims to suppress appetite and increase energy expenditure. Metformin is a biguanide that lowers blood glucose by inhibiting hepatic gluconeogenesis and improving peripheral insulin sensitivity, primarily through the activation of AMP-activated protein kinase (AMPK). The combination is being evaluated in Phase 2 clinical trials for the treatment of type 2 diabetes and obesity, potentially offering enhanced glycemic control and weight loss compared to monotherapy.
