Susceptibility-guided tailored therapy (Group A) is a personalized antimicrobial treatment protocol developed by National Taiwan University Hospital for the rescue treatment of *Helicobacter pylori* infection. Unlike standard empiric therapy, this approach selects one of five specific antibiotic regimens based on the results of antimicrobial susceptibility testing (AST) and Minimum Inhibitory Concentration (MIC) profiling of the patient's specific *H. pylori* strain. The potential regimens include clarithromycin triple therapy, levofloxacin triple therapy, metronidazole triple therapy, high-dose dual therapy, or bismuth quadruple therapy. Each regimen incorporates the proton pump inhibitor rabeprazole to suppress gastric acid, thereby optimizing the stability and efficacy of the co-administered antibiotics (amoxicillin, clarithromycin, levofloxacin, metronidazole, tetracycline, or bismuth). This tailored strategy is designed to achieve higher eradication rates by specifically addressing the antibiotic resistance profile of the infecting bacterial strain.
