SYM004 + cisplatin + pemetrexed is a combination regimen under investigation for the treatment of various cancers, particularly non-small cell lung cancer (NSCLC) and other solid tumors. SYM004 is a first-in-class synergistic antibody mixture composed of two recombinant monoclonal antibodies, futuximab and modotuximab, which bind to different, non-overlapping epitopes on the extracellular domain III of the epidermal growth factor receptor (EGFR). This dual binding induces highly efficient internalization and degradation of EGFR on cancer cells, leading to potent inhibition of EGFR signaling pathways. Unlike single anti-EGFR antibodies such as cetuximab or panitumumab, SYM004 can overcome resistance mediated by EGFR mutations or increased ligand production[1][2][3][5]. Cisplatin is a platinum-based chemotherapy agent that forms DNA crosslinks and inhibits DNA synthesis in rapidly dividing cells. Pemetrexed is an antifolate chemotherapeutic that disrupts folate-dependent metabolic processes essential for cell replication. The combination with SYM004 aims to enhance antitumor efficacy through complementary mechanisms: targeted inhibition of EGFR-driven signaling by SYM004 alongside cytotoxic effects from cisplatin and pemetrexed.
