SYM004 + FOLFIRI is a combination regimen comprising SYM004—a 1:1 mixture of two monoclonal antibodies targeting non-overlapping epitopes of the Epidermal Growth Factor Receptor (EGFR)—and the chemotherapy regimen FOLFIRI, which includes folinic acid (leucovorin), fluorouracil (5-FU), and irinotecan. SYM004 is designed to inhibit EGFR signaling, which is often upregulated in colorectal cancer, thereby blocking cell proliferation and survival pathways. FOLFIRI acts through combined cytotoxic mechanisms: fluorouracil inhibits DNA synthesis by targeting thymidylate synthase, irinotecan inhibits topoisomerase I leading to DNA damage, and folinic acid enhances the binding and efficacy of fluorouracil. The combination has been investigated in advanced/metastatic colorectal cancer, particularly in patients progressing after first-line therapy, in open-label Phase 1b/2a clinical trials.
