SYN023 + rabies vaccine is a combination therapy for post-exposure prophylaxis (PEP) of rabies, consisting of a **1:1 mixture of two anti-rabies humanized monoclonal antibodies** (CTB011 and CTB012)—collectively formulated as SYN023—administered together with a standard rabies vaccine. CTB011 and CTB012 are IgG1κ antibodies that bind to non-overlapping epitopes of the rabies virus glycoprotein G, providing broad-spectrum neutralization against diverse rabies virus strains. This antibody cocktail is designed as a safer and more accessible alternative to human rabies immune globulin (HRIG), with a mechanism that neutralizes the rabies virus through high-affinity binding, likely also enabling Fc-mediated effector functions like complement-dependent cytotoxicity. The rabies vaccine stimulates an active immune response. This combination is intended to provide both **immediate passive immunity** and longer-term active immunity following rabies exposure, targeting Category III rabies exposures per WHO classification. The combination is under late-stage clinical evaluation for efficacy and safety in comparison to HRIG.
