SYS6090 + bevacizumab + mFOLFOX6 is an investigational combination therapy regimen developed by CSPC Megalith Biopharmaceutical for the treatment of advanced colorectal cancer, specifically targeting patients with proficient mismatch repair or microsatellite stable (pMMR/MSS) tumors. The regimen integrates SYS6090, a recombinant humanized monoclonal antibody directed against Claudin 18.2 (CLDN18.2), with the anti-angiogenic agent bevacizumab and the standard-of-care chemotherapy backbone mFOLFOX6 (modified FOLFOX6). SYS6090 binds to CLDN18.2, a protein often overexpressed in gastrointestinal malignancies, to facilitate tumor cell death through mechanisms such as antibody-dependent cellular cytotoxicity (ADCC). Bevacizumab targets vascular endothelial growth factor A (VEGF-A) to inhibit tumor angiogenesis. The mFOLFOX6 component, consisting of oxaliplatin, leucovorin, and 5-fluorouracil, provides cytotoxic effects by inducing DNA cross-linking and inhibiting thymidylate synthase. This multi-targeted approach is designed to overcome the limited efficacy of single-agent therapies in MSS colorectal cancer by combining targeted immune activation, anti-angiogenesis, and cytotoxic chemotherapy.
