Syzygium cumini + momordica charantia + wrightia tinctoria + gymnema sylvestre is a polyherbal anti-diabetic formulation containing equal proportions of these four medicinal plants. Developed by Dr. JRK's Siddha Research and Pharmaceuticals, it is primarily indicated for the management of hyperglycemia in patients with Type 2 Diabetes Mellitus. The formulation exerts its therapeutic effects through a multi-targeted approach: *Gymnema sylvestre* contains gymnemic acids that inhibit intestinal glucose absorption via SGLT1 and stimulate insulin secretion from pancreatic beta cells; *Momordica charantia* provides insulin-mimetic peptides (polypeptide-p) and activates AMPK; *Syzygium cumini* inhibits carbohydrate-digesting enzymes such as alpha-glucosidase and alpha-amylase; and *Wrightia tinctoria* contributes to the overall hypoglycemic effect. This combination was evaluated in a Phase III clinical trial (CTRI/2018/03/012449), where it demonstrated efficacy comparable to Metformin in reducing HbA1c and fasting blood glucose levels with a favorable safety profile.
