This combination treatment regimen, investigated in the TALEM trial (NCT05555316) by Sun Yat-sen University Cancer Center, is designed for patients with large unresectable hepatocellular carcinoma (HCC) (≥7 cm). The regimen consists of modified transarterial chemoembolization using blank microspheres with low-dose chemotherapy (BMS-TACE), followed by oral administration of the multi-kinase inhibitor lenvatinib, and sequential radical microwave ablation (MWA) once downstaging is achieved. Lenvatinib targets VEGFR1-3, FGFR1-4, PDGFRα, RET, and KIT to inhibit angiogenesis and tumor cell proliferation. The TACE component typically involves lipiodol, low-dose doxorubicin, and lobaplatin. The regimen aims to convert unresectable large tumors into candidates for curative ablation, showing high objective response and downstaging rates in clinical studies.
