Tacrolimus acts by binding to the intracellular protein FKBP12, while cyclosporine A binds to cyclophilin A; both resulting complexes inhibit the phosphatase activity of calcineurin. This inhibition prevents the dephosphorylation and nuclear translocation of the nuclear factor of activated T-cells (NFAT), thereby blocking the transcription of interleukin-2 (IL-2) and other cytokines essential for T-lymphocyte activation. The specific study investigates the feasibility of inducing immune tolerance by gradually tapering these medications to identify biomarkers that predict successful withdrawal without graft rejection.
