**Tacrolimus + FK778** is a combination of two immunosuppressive agents: tacrolimus (a calcineurin inhibitor widely used in organ transplantation to prevent rejection) and FK778 (also known as malononitrilamide 715; a synthetic low molecular weight immunosuppressive agent). Tacrolimus acts mainly by inhibiting T-cell activation via calcineurin blockade, while FK778 inhibits T- and B-cell proliferation, phagocyte function, and displays antiviral activity (notably against cytomegalovirus). Their combination demonstrates synergistic or additive immunosuppressive effects, particularly in preclinical models of allograft transplantation, significantly increasing graft survival compared to monotherapy.
