This regimen combines **tadalafil**, a phosphodiesterase type 5 (PDE5) inhibitor, with an **alpha1 blocker** (such as alfuzosin or tamsulosin). Tadalafil acts by inhibiting PDE5, resulting in smooth muscle relaxation in the prostate, bladder, and penile tissues, improving urinary and erectile function. Alpha1 blockers antagonize adrenergic alpha1 receptors on smooth muscle in the lower urinary tract, leading to decreased sympathetic tone and improved urine flow. The combination demonstrates additive effects in relaxing detrusor and prostatic smooth muscle, improving symptoms of LUTS and ED.
