Tafolecimab + sintilimab + etoposide + carboplatin + cisplatin is a combination therapy regimen being evaluated as a first-line treatment for extensive-stage small cell lung cancer (ES-SCLC). The regimen combines tafolecimab, a recombinant fully humanized monoclonal antibody targeting proprotein convertase subtilisin/kexin type 9 (PCSK9), with sintilimab, a PD-1 checkpoint inhibitor, and standard chemotherapy (etoposide and a platinum agent, either carboplatin or cisplatin). Beyond its established role in lipid metabolism, PCSK9 inhibition is hypothesized to enhance the expression of major histocompatibility complex class I (MHC-I) on tumor cells, potentially overcoming resistance to immunotherapy and synergizing with PD-1 blockade. This multi-modal approach is currently being investigated in the STAR-SCLC trial, a Phase 2 study sponsored by the Second Affiliated Hospital of Zhejiang University.
