Overview

Tafolecimab + sintilimab + SOX is an investigational combination regimen being evaluated for the treatment of proficient mismatch repair (pMMR) or microsatellite stable (MSS) advanced gastric and gastroesophageal junction adenocarcinoma. The regimen consists of **tafolecimab**, a fully humanized monoclonal antibody targeting PCSK9; **sintilimab**, a monoclonal antibody targeting the PD-1 receptor; and **SOX chemotherapy**, which is a combination of S-1 (a fluoropyrimidine prodrug) and oxaliplatin. The rationale for this combination is based on preclinical evidence suggesting that PCSK9 inhibition can enhance anti-tumor immunity by increasing the cell surface expression of MHC class I molecules, thereby improving antigen presentation and sensitizing immunologically "cold" tumors to PD-1 blockade and chemotherapy.
