Tagraxofusp + venetoclax is an investigational combination therapy under clinical study primarily for aggressive hematologic malignancies, specifically blastic plasmacytoid dendritic cell neoplasm (BPDCN) and acute myeloid leukemia (AML). Tagraxofusp is a recombinant fusion protein composed of interleukin-3 fused to a truncated diphtheria toxin payload; it targets CD123, leading to targeted cell ablation. Venetoclax is a small molecule inhibitor of BCL-2, promoting apoptosis in malignant cells. The rationale for combining these agents is that tagraxofusp targets CD123-positive cells, while venetoclax inhibits anti-apoptotic pathways, potentially overcoming resistance mechanisms. The combination is being investigated alone and with azacitidine, a DNA hypomethylating agent, to enhance efficacy in CD123-positive malignancies and in settings of adverse genetics such as TP53 mutations[3][2][6][7][4].
