**TAK-242 + filgrastim** is an investigational combination therapy currently in phase 2 clinical trials for acute-on-chronic liver failure (ACLF), particularly in the setting of severe alcoholic hepatitis. TAK-242 (also known as resatorvid) is a small molecule selective inhibitor of **Toll-like receptor 4 (TLR4)-mediated signaling**, designed to reduce systemic inflammation and prevent immune "over-reaction." Filgrastim (also known as G-CSF, or granulocyte-colony stimulating factor) is a recombinant human granulocyte colony-stimulating factor that promotes the proliferation and mobilization of hematopoietic progenitor cells, thereby encouraging hepatic regeneration. Preclinical studies in animal models show that their combination both inhibits inflammation (via TAK-242) and promotes regeneration (via filgrastim), leading to improved outcomes over either agent alone[1][3][5].
