talquetamab + dexamethasone

Tal-D is a combination therapy regimen comprising talquetamab (Talvey) and dexamethasone, primarily used for the treatment of relapsed or refractory multiple myeloma. Talquetamab is a first-in-class bispecific T-cell engaging antibody that simultaneously binds to G protein-coupled receptor family C group 5 member D (GPRC5D) on plasma cells and CD3 on T cells, leading to T-cell activation and subsequent lysis of GPRC5D-expressing myeloma cells. Dexamethasone, a potent corticosteroid, is administered alongside talquetamab to enhance its anti-tumor activity and to mitigate treatment-related inflammatory responses, such as cytokine release syndrome (CRS). Developed by Janssen, this combination has demonstrated significant efficacy in patients who have exhausted standard therapies, including proteasome inhibitors, immunomodulatory agents, and anti-CD38 monoclonal antibodies.
