**Talquetamab + teclistamab** is a combination of two first-in-class bispecific monoclonal antibodies developed for the treatment of relapsed or refractory multiple myeloma, particularly in patients who have received prior lines of therapy including proteasome inhibitors, immunomodulators, and anti-CD38 antibodies. Talquetamab targets the G protein-coupled receptor family C group 5 member D (GPRC5D) and redirects T cells to kill multiple myeloma cells by binding CD3, while teclistamab similarly redirects T cells but targets B-cell maturation antigen (BCMA). By dual targeting of two distinct myeloma-associated antigens, the combination is hypothesized to reduce resistance mechanisms and enhance antitumor activity. Clinical results from the RedirecTT-1 trial show high overall response rates, deep and durable responses, and activity in high-risk populations, including those with extramedullary disease. However, the regimen is associated with significant risks of infections, cytokine release syndrome, and other hematologic toxicities[1][3][4][5][7][9].
