**Tarlatamab + atezolizumab** is a combination regimen under clinical investigation for the treatment of extensive-stage small cell lung cancer (ES-SCLC) as maintenance therapy following initial chemo-immunotherapy. Tarlatamab is a bispecific T-cell engager (BiTE) antibody designed to bridge T cells to tumor cells by binding both delta-like ligand 3 (DLL3), which is highly expressed on SCLC cells, and CD3, a component of the T-cell receptor complex, thereby activating T-cell mediated tumor lysis. Atezolizumab is a monoclonal antibody targeting programmed cell death ligand 1 (PD-L1), functioning as an immune checkpoint inhibitor to promote anti-tumor immune activity. This combination is aimed at enhancing anti-tumor efficacy by simultaneous direct engagement of tumor-directed cytotoxicity (via tarlatamab) and checkpoint blockade (via atezolizumab). The regimen is being tested for safety and preliminary efficacy in ongoing phase 1b and phase 3 studies for ES-SCLC maintenance[1][3][4][7].
