Overview

This is a combination therapy consisting of four agents: **tarlatamab**, **durvalumab**, **carboplatin**, and **etoposide**, investigated as a first-line treatment for patients with extensive-stage small cell lung cancer (ES-SCLC). - **Tarlatamab** is a bispecific T-cell engager (BiTE) antibody targeting delta-like ligand 3 (**DLL3**) on SCLC cells and **CD3** on T cells to direct T-cell cytotoxicity toward DLL3-expressing tumor cells. - **Durvalumab** is a human monoclonal antibody and immune checkpoint inhibitor that binds programmed death-ligand 1 (**PD-L1**), restoring T-cell activity and promoting immune-mediated tumor cell death. - **Carboplatin** is a platinum-based small molecule chemotherapy agent that forms DNA crosslinks, resulting in apoptosis of dividing cells. - **Etoposide** is a topoisomerase II inhibitor that impedes DNA replication and transcription, causing DNA strand breaks and cancer cell death. This combination is being studied in the phase 3 DeLLphi-305 trial for its ability to improve outcomes compared to standard treatment regimens including platinum/etoposide and immunotherapy in SCLC[1][5][7].
